PLEASE READ: Athletic Eligibility Requirements
The following are REQUIRED and need to be completed and turned in to the TJMS office PRIOR to the first day of practice:
> A physical examination taken April 1 and after is valid for the following two school years. (i.e: a physical dated 4/1/24 and after is good for both 24-25 & 25-26 school yrs)
> A physical examination taken before April 1 is valid only for the remainder of that school year and the following school year.
> In the year when an examination is not required, an Alternate Year Athletic Permit Card must be signed by a parent/guardian and be on file.
2. Please read Concussion Information and then complete the Concussion Form.
3. Paid activity fee ($60 per school year / $120 maximum per family) must be paid before participation.
If your child participated in inter-scholastic sports last year and you would like to check if a physical will be needed (or an alternate card) for this year, please contact Scott Parsons scott.parsons@pwssd.k12.wi.us
